Hurricane Season Back Pain: When Storm Prep Hurts Your Spine

September is peak hurricane season in Southwest Florida. The prep work that protects your home can wreck your back. Here is what actually injures spines during storm prep, and what to do when it happens to you.

The alert comes in at 9pm on a Tuesday. A tropical system is tracking toward the Tampa Bay area and landfall is 36 to 48 hours out. By Wednesday morning, Lakewood Ranch is in motion: hardware store lines wrap around the building, outdoor furniture is getting stacked against garage walls, and generators are being dragged out of storage for the first time since last season.

By Thursday, a predictable second wave hits our area. Not the storm itself. The patients calling with acute low back pain that started mid-prep. A generator lift gone wrong. A few too many sandbag passes. A rushed attempt to push a 200-pound concrete planter to the side of the house. The physics of hurricane prep are hard on the lumbar spine, and the urgency of the situation makes people skip the technique that would normally protect them.

This post covers why storm prep is so hard on backs, what types of injuries happen most often, and what to do from the moment something goes wrong.

Why storm prep is a spine injury waiting to happen

The physical tasks involved in securing a home before a hurricane share three traits that spine specialists recognize as a high-risk combination:

  • Load plus awkward angle. A portable generator weighs 70 to 250 pounds depending on the model. Moving it typically involves low-angle pulling or lifting from a garage shelf, a storage shed, or the back of a truck. The lumbar spine under combined compressive load and forward flexion is exactly the scenario that bulges discs.
  • Repetition without rest. Filling and stacking sandbags is not a single heavy lift. It is 50 or 100 moderate lifts in the same bending-and-stacking pattern, which fatigues the posterior spinal muscles and progressively removes their protective function. The first 20 bags feel fine. The injury often happens on bag 45.
  • Time pressure. Normal people stop when something hurts. Under a storm deadline, they push through. "I can rest after" is a direct line to a three-day flare and a week of lost function.

Add Florida's September heat, which accelerates dehydration (dehydrated spinal discs are mechanically stiffer and more vulnerable to injury), and the risk profile rises further.

The injuries we see most after a storm prep weekend

Three injury patterns show up repeatedly in the weeks following a major storm event in Manatee and Sarasota counties:

Lumbar muscle strain

The most common. The erector spinae, multifidus, and quadratus lumborum muscles along the lower back are overloaded, micro-tearing in the muscle belly or at the attachment. Pain is typically dull-to-sharp, localized to one or both sides of the lower back, worse with movement, and eases somewhat with lying still. Most muscle strains respond well within 1 to 3 weeks with appropriate care. The problem is that people who ignore a strain and continue heavy activity through it can turn a simple strain into a structural problem.

Disc injury (bulge or herniation)

The most serious. When a heavy load compresses the spine while it is in flexion (the classic "bent over lifting" position), the nucleus pulposus (the gel-like center of the disc) can push through the annulus fibrosis (the outer disc ring). A bulge is a partial push. A herniation is a full breach. Both can press on nerve roots and create sciatica: pain, numbness, or tingling that shoots down one leg, sometimes all the way to the foot. A disc injury that produces neurological symptoms (leg weakness, foot drop, bladder or bowel changes) is a medical emergency. For a disc injury with pain only, the first step is determining which disc is involved and how much the nerve is affected. Sacroiliac joint sprain

The sacroiliac (SI) joint connects the sacrum at the base of the spine to the ilium of the pelvis. It takes significant shear force during heavy lifting, especially when the load is shifted to one side (think: dragging something heavy). SI sprains produce a sharp, sometimes debilitating pain low in the back and slightly off-center, often confused with disc pain. The distinction matters clinically because the approach is different.

In 23 years of practice, the heaviest lifts are rarely what brings someone in. It is the 47th sandbag, or the generator they pulled out of an awkward corner. Fatigue changes biomechanics in ways that a single maximal effort usually does not.

What to do in the first 24 to 48 hours

The hours immediately after a storm prep injury matter. Here is what to do and what to skip:

  • Ice, not heat, for acute injury. In the first 48 to 72 hours, swelling and inflammation are the primary problem. Ice (10 to 15 minutes on, at least 45 minutes off) slows that process. Heat increases blood flow and increases swelling in acute injury. Reverse the rule after 72 hours, when increasing circulation helps tissue repair.
  • Move, but carefully. Complete rest is no longer the recommended approach for most back injuries. Short, gentle walks keep circulation moving and prevent the spasm cycle that sets in when injured muscles are completely immobilized. Do not stay in bed for days; do not try to push through heavy activity.
  • OTC anti-inflammatories if you can take them. Ibuprofen or naproxen reduce acute inflammation. Follow label dosing and talk to your pharmacist if you have stomach or kidney concerns.
  • Do not try to "stretch it out" aggressively. Aggressive forward bending or twisting in an acutely injured spine can worsen a disc injury. Gentle movement is appropriate. Forcing range of motion is not.
  • Get evaluated if there is any leg pain, numbness, or tingling. Those symptoms mean a nerve is involved, and that changes the urgency of the evaluation significantly.

When back pain from storm prep needs professional care

Most muscle strains improve meaningfully within 72 hours of appropriate initial management. If yours is not improving, or if any of these are present, get a professional evaluation sooner rather than later:

  • Pain that shoots into the buttock or down either leg
  • Numbness or tingling in the legs or feet
  • Pain that is worse lying down than standing
  • Any weakness in the legs, especially difficulty lifting the foot (foot drop)
  • Bladder or bowel changes (go to an emergency department immediately)
  • Pain that was tolerable at first but is worsening after 24 to 48 hours
  • Pain that is sharp and severe with any attempt to stand upright

Waiting out back pain that has a nerve component typically extends the recovery timeline, not shortens it. For disc injuries and herniated disc cases specifically, the first 4 to 6 weeks after injury are the window when conservative care has the best results. After that, scar tissue and compensation patterns set in and make everything harder.

How we evaluate a storm prep back injury

When someone comes in after a hurricane prep injury, Dr. Banman's evaluation covers several things that determine what structure is actually involved:

  • A full history of exactly what happened, the position involved, and how the pain has progressed
  • Orthopedic tests that distinguish muscle, disc, facet, and SI joint involvement (each has a different pain pattern and a different provocation sign)
  • Neurological screen for radiculopathy: reflexes, sensation, motor strength in the lower extremities
  • Postural and movement analysis to identify compensation patterns that increase re-injury risk

Based on the findings, the plan might involve chiropractic care to restore spinal mechanics, decompression for disc involvement, Class IV laser therapy to reduce inflammation and speed soft-tissue repair, or EMS to address muscle guarding. Many storm prep injuries resolve with a focused course of conservative care. Some need imaging before we proceed. A few need a specialist referral. The evaluation determines which path fits.

Protecting your spine when the next storm prep window comes

September is not the only hurricane month. October and early November still carry risk. When the next prep window arrives, a few technique adjustments go a long way:

  • Team lift generators. There is no back-saving way to solo-lift a 150-pound generator. Two people with a plan is the only safe approach. If you are alone, use a hand truck or appliance dolly.
  • Stage sandbag work in sessions. Fill 20 bags, rest 10 minutes. Fill 20 more. The acute fatigue that accumulates from non-stop filling is what drops the back muscles' ability to protect the disc.
  • Bring the load close before lifting. The moment a load is more than 6 to 8 inches from your center of gravity, the mechanical disadvantage triples. Slide the object toward you first, then lift.
  • Hinge at the hips, not the waist. The distinction matters mechanically. Waist bending puts the spine in forward flexion under load. Hip hinging keeps the spine neutral. Most people have to practice this movement before it becomes automatic.
  • Hydrate the day before prep begins. Spinal discs are 80% water. Dehydration reduces their shock-absorbing capacity. Starting prep already well-hydrated is one of the simplest protective steps.

These are not complicated adjustments. But under deadline pressure, in the heat, trying to beat a storm, the basics go out the window fast. Having the plan in your head before the alert comes makes it more likely you actually use it.
