Shoulder Bursitis: Why Your Shoulder Aches Even When You Cannot Point to an Injury

Your shoulder aches when you reach up, throbs at night, and is tender when pressed near the top. No fall, no obvious injury. Shoulder bursitis is one of the most under-recognized causes of exactly that pattern.

Most people who come into our Lakewood Ranch office with shoulder bursitis have already been living with it for weeks. They have chalked it up to sleeping on it wrong, a strained muscle, or just getting older. The shoulder does not pop. It does not lock up. It just aches, especially when reaching overhead, and it gets noticeably worse at night when lying on that side.

That pattern, aching without a clear mechanical injury, tenderness near the top of the shoulder, and night pain that disrupts sleep, is a fairly specific presentation. Once you understand what the bursa is and why it inflames, the symptoms make a lot of sense. And once you know what actually settles it down, the path forward is clearer than most people expect. What Is the Bursa and Why Does It Inflame?

A bursa is a small, fluid-filled sac. Your body has around 150 of them. Their job is to reduce friction between tendons, bones, and muscles that slide past each other during movement. The shoulder has several bursae, but the one that causes most of the trouble is the subacromial bursa. It sits between the rotator cuff tendons and the underside of the acromion (the bony shelf at the top of your shoulder).

When the bursa is healthy, it is thin and almost invisible on imaging. When it is irritated or compressed repeatedly, it fills with fluid and thickens. That thickened, swollen sac now takes up space in a joint that does not have much space to give. Every time you lift your arm, the swollen bursa gets pinched between the rotator cuff tendons and the acromion. That pinching creates sharp pain on elevation and a dull, deep ache at rest.

Common triggers include:

  • Repetitive overhead motion (painting, stocking shelves, pickleball, swimming)
  • A direct fall on an outstretched arm or onto the shoulder tip
  • Rotator cuff weakness that allows the humeral head to ride slightly too high
  • Bone spurs on the underside of the acromion (a structural issue that narrows the space)
  • Age-related tendon changes that reduce the available space in the subacromial canal

Notice that "I did not do anything" is not on that list. Bursitis often develops gradually from cumulative compression rather than a single event. That is why patients are frequently surprised to get this diagnosis.

How Shoulder Bursitis Feels: The Typical Pattern

The symptom pattern is distinctive enough that an experienced clinician can often make a working diagnosis before imaging. That said, a physical exam is still essential to rule out things that can look similar but need different management.

Most patients with subacromial bursitis describe:

  • Pain on the outside of the upper arm, roughly where you might grab your arm if you were nursing a wound. The pain is usually not localized to a specific spot but feels deep and diffuse in that general zone.
  • A painful arc when raising the arm. Pain typically starts around 60 to 90 degrees of elevation and then eases if you continue raising the arm all the way overhead. This painful arc is a hallmark finding.
  • Tenderness to pressure on or just below the top of the shoulder, particularly in the small space below the bony shelf. Pressing there reproduces a dull, sickening ache.
  • Night pain that is noticeably worse when lying on the affected side. The lateral position compresses the bursa and increases fluid pressure. Many patients say the shoulder is manageable during the day and much worse once they try to sleep.
  • Pain reaching behind the back (fastening a bra, reaching into a back pocket) rather than just overhead.
The painful arc is the most telling sign. If your shoulder hurts most between 60 and 90 degrees of elevation but feels better when you push through and raise it fully overhead, that implicates the subacromial space. A truly frozen shoulder hurts throughout the entire range and restricts it significantly. That distinction matters for what comes next.

Bursitis vs. Other Common Shoulder Problems

Shoulder bursitis rarely exists in isolation. The bursa sits right next to the rotator cuff tendons, and the same impingement that irritates the bursa often affects those tendons too. Distinguishing between them matters because the treatment emphasis shifts slightly.

Bursitis vs. rotator cuff tendinitis: Both cause the painful arc and night pain. Tendinitis tends to produce a sharper, more localized pain over the specific tendon involved (usually the supraspinatus, toward the top front of the shoulder). Bursitis tends to produce a deeper, more diffuse ache. In practice, many shoulder impingement presentations involve both. Bursitis vs. frozen shoulder (adhesive capsulitis): Frozen shoulder restricts motion in all directions and is usually quite severe. Bursitis restricts painful elevation but typically preserves most passive range of motion. If a provider can move your arm through a full range of motion when you are relaxed, bursitis is far more likely than frozen shoulder. Bursitis vs. shoulder impingement syndrome: These terms overlap considerably. "Impingement" describes the mechanism (the bursa and tendons getting pinched). "Bursitis" describes the tissue response (the bursa swelling). Most patients have both. Our shoulder impingement post covers what triggers the pinching in the first place.

Bursitis vs. AC joint arthritis: AC joint pain is usually localized to the very top of the shoulder where the collarbone meets the acromion. Cross-body adduction (bringing the arm across the chest toward the opposite shoulder) tends to worsen AC joint pain but may not significantly affect bursitis. An exam can differentiate the two.

What Makes Shoulder Bursitis Worse

Understanding what aggravates the bursa helps you protect it while care progresses. The three biggest drivers are:

Repeated overhead work. Any activity that requires lifting the arm above shoulder height compresses the subacromial space. Painting ceilings, loading overhead cabinets, swimming freestyle, serving in tennis or pickleball, and overhead pressing in the gym all fall here. These activities do not need to be heavy to irritate the bursa. Repetition is the issue.

Sleeping on the affected side. Gravity, body weight, and the lateral position all conspire to compress the bursa overnight. Many patients with shoulder bursitis sleep in the same position out of habit and wake up significantly sorer than they were before bed.

Reaching behind the back. Internal rotation in the shoulder, especially combined with extension (reaching into a back pocket, hooking a bra, tucking in a shirt), brings the humeral head into a position that compresses the bursa from a different angle. Many patients are surprised this specific movement hurts more than overhead motion.

How We Evaluate and Treat Shoulder Bursitis

The exam for suspected bursitis typically includes range-of-motion testing, specific orthopedic tests (Neer, Hawkins-Kennedy, and the empty can test for the supraspinatus), and palpation of the subacromial space and surrounding structures. In some cases we order imaging: X-ray to assess the acromion shape and rule out bone spurs or arthritis; MRI if we suspect significant rotator cuff involvement or want to visualize the bursa directly.

Treatment at our Lakewood Ranch office depends on how inflamed the bursa is, how long it has been present, and what is driving the impingement. A freshly irritated bursa responds differently than one that has been swollen for months.

Chiropractic adjustments to the shoulder and cervical spine. Joint dysfunction in the shoulder complex (acromioclavicular joint, glenohumeral joint) alters the mechanics of how the humerus moves through elevation. Restoring normal joint motion reduces unnecessary compression on the subacromial space. Class IV laser therapy. Our Class IV laser delivers energy deep into inflamed tissue to reduce swelling, promote cellular repair, and modulate pain. Many patients report noticeable improvement in the sharp, arc-related pain after a few sessions. Class IV laser is one of our most-used tools for soft-tissue shoulder problems because it reaches the bursa depth that surface treatments cannot.

Shockwave or softwave therapy. For bursitis that has become chronic or is associated with calcific deposits in the tendon, acoustic wave therapy can break up calcification and stimulate tissue remodeling. It is more aggressive than laser but tends to work well on stubborn cases.

Postural and biomechanical correction. Many patients with subacromial impingement have rounded shoulders and forward head posture, which tilts the acromion downward and narrows the subacromial space structurally. Correcting that postural pattern is part of the long-term fix, not just the short-term symptom control.

Activity modification during the acute phase. We ask patients to avoid the specific movements that compress the bursa while inflammation is active. That is not the same as complete rest. Gentle, pain-free range-of-motion work keeps the shoulder from stiffening while the bursa calms down.

What You Can Do at Home Right Now

A few things genuinely help while you are waiting for an appointment or early in the treatment process:

  • Ice the subacromial area for 15 minutes, 2 to 3 times a day. Keep a thin cloth between the ice pack and your skin. Ice does not eliminate bursitis but it can reduce the acute inflammatory flare that makes the first few days miserable.
  • Sleep on your back or on the opposite side. Prop the affected arm on a pillow at about 45 degrees of abduction (angled out from the body) if you must stay on your back. This reduces the pressure on the subacromial space overnight.
  • Avoid reaching overhead and behind your back for overhead activities that you can defer. Use a step stool instead of stretching to reach high shelves. Ask for help with tasks that require the arm behind the back. This is temporary, not permanent.
  • Gentle pendulum exercises. Standing, bend forward at the hips and let the affected arm hang freely. Swing it gently in small circles, then front-to-back and side-to-side. These use gravity to create gentle traction in the joint, which can ease fluid pressure and maintain mobility without compressing the bursa.

When to Expect Results

A case of bursitis caught early, within the first few weeks of symptoms, typically responds well to conservative care. Many patients in this situation see meaningful improvement over 4 to 8 weeks of consistent treatment combined with activity modification.

Chronic bursitis that has been present for months or has calcific changes is a different situation. The bursa itself may be thickened to the point where passive treatment has limits. In those cases we discuss options including steroid injection referral (to temporarily reduce bursa volume and allow more aggressive rehabilitation) or imaging to rule out a structural rotator cuff tear that may need surgical consultation.

The important thing is not to ignore a shoulder that has been aching for months. Bursitis that goes unaddressed does not tend to improve on its own, and compensatory movement patterns developed to protect the painful shoulder can create secondary problems in the neck, upper back, and opposite shoulder.

When to Seek Urgent Care Instead

Shoulder bursitis, while painful, is not a medical emergency. However, a few presentations warrant prompt evaluation beyond a routine appointment:

  • Fever combined with a hot, swollen, visibly red shoulder (possible septic bursitis, which requires antibiotics)
  • Sudden inability to lift the arm at all after a specific event, particularly in someone over 60 (possible complete rotator cuff tear)
  • Chest or left arm pain radiating from the shoulder (rule out cardiac cause first)
  • Numbness or weakness in the hand or fingers (possible cervical nerve involvement)
